These 3 items are required for ALL STUDENT ATHLETES in order to play a sport at St. John Paul the Great:
- A fully completed Diocese of Arlington Sports Physical (signed by parent, student-athlete, and physician) uploaded to the Athletic Trainer System (ATS). For more information on how to do this, click HERE. We are NO longer accepting VHSL or VISAA sports physical forms. Please ONLY use the new sports physical form approved by the Diocese.
- Complete the Injury & Concussion Acknowledgement Form and electronically sign on ATS. This is located in the “Paperwork” tab after clicking “Your Information”. Click the “Complete Form” box. Both athlete and parent must read and electronically sign the document and save to your ATS profile. This form fulfills Virginia law and Diocese policy regarding concussion education, reporting, and return-to-play protocol.
- Fill out the JP Athletics Team Commitment & Expectation Contract and electronically sign on ATS. This is located on the same page as the Injury & Concussion Acknowledgement Form (see #2) on ATS.
These 2 additional items are ONLY for FRESHMAN, NEW TRANSFERS, and JUNIORS:
- Go to PaySimple to pay the $30 fee for an ImPact Concussion Baseline Test.
- Take the Computerized ImPact Concussion Baseline Test at school before strenuous exercise. Athletes only need to take this once every 2 years.

at 5pm (Cross Country)
No need to register, athletes just pay the $30 fee on PaySimple. Please make sure the fee is paid first before sitting for the baseline test. Athletes bring the receipt to the testing session.

Medical matters related to sports participation are managed by our Athletic Trainers for the hundreds involved in JP Athletics each season all year. If a coach cannot provide an answer, contact our ATs by email:
