Attention Athletes and Parents! 

Students interested in playing sports at JP for the 2026-2027 school year may now begin scheduling sports physical appointments with their healthcare providers!

The Catholic Diocese of Arlington has created a NEW 5 page sports physical form to be used across all high schools. We are NO longer accepting Virginia High School League (VHSL) or Virginia Independent Schools Athletic Association (VISAA) sports physical forms. The 5-page form must be completed in its entirety and signed by the student, a parent/guardian, and a licensed healthcare provider (MD, DO, Nurse Practitioner, or Physician Assistant) before uploading it to Athletic Trainer System (ATS).

NOTE: The sports physical exam must be completed after May 1st, 2026 in order to be accepted for the 2026-2027 school year. All 2025-2026 sports physicals expire June 30, 2026. Parents are encouraged to schedule appointments before the deadline if athlete plans to attend summer workouts or participate in fall sports.
